Media Consent Form

In celebrating, remembering and telling the stories of our churches and associated children and youth groups and events, it is invaluable to use pictures in contemporary media.

Please be aware that Momentum Church Facebook and Website is a public domain.

We ask for your agreement that yourself and/or your child/ren may be included while you participate in activities organised by Momentum Church Ltd.

Conditions of Use:

1. This form is:
   1. Valid for all images / videos taken during one year from the date below;
   2. Valid for all activities and events held by Momentum Church Ltd, not limited to children’s ministry and/or youth group, but general services as well; and
   3. Valid for all types of media including (but not limited to) photos, audio recordings, video recordings, social media, and the like. 
2. We will not capture any images after this time period without asking you to renew your consent, although we may later re-use the photos/videos taken during the period of this Consent.
3. We will not include details of full names (which means first and last names) of any child, their parent/s or adult in a photo, video, website, or printed publications, without consent. For example, we may include the full name of an interviewee or award winner if we have their consent.
4. We will not include personal email or postal addresses, telephone, mobile phone or fax numbers on any of our material without your consent.
5. If we use images of individuals, we will not use the name of that individual in the accompanying text or caption without good reason and consent. If the individual is named in the text, we will not use the photograph to accompany the article without good reason and consent.
6. We may use group images with general labels.
7. We will only use images of young people suitably dressed to reduce the risk of images being used inappropriately.
8. We may share photos/videos between Momentum Church Ltd’s communities and ministerial departments only.
9. We fully understand if you wish to refrain from giving your permission and we will respect your choice.
